The DOJ has launched a sweeping fraud investigation into UnitedHealth for allegedly conning the government out of millions through fraudulent billing practices. The new civil fraud probe is said to involve a review of the company's practices for recording diagnoses that trigger extra payments to its Medicare Advantage plans. A Wall Street Journal investigation showed that Medicare has paid UnitedHealth to the tune of millions of dollars for questionable diagnoses in recent years.
